Shaped and Textured Implants are Next Generation for Breast Enhancement

WSVN.com recently posted a story about “Aimee,” a woman in South Florida who just had breast augmentation surgery with shaped silicone gel implants.

Dr. Blane Shatkin appears on the segment to discuss the new implants, which feature a natural teardrop shape and cohesive silicone gel. “You don’t want to put them in upside down or sideways” he says and “you don’t want them to move in the implant pocket.”

Shaped implants are textured to help prevent rotation. When used in breast augmentation, the implants provide less implant height and greater projection, the doctor explains.

Aimee was pleased with the natural results of her breast augmentation. “It gave me a little more fullness on top,” she said. Her new bathing suit fits perfect, reports WSVN News.

Shaped Silicone Gel Breast Implants by Sientra

Their shaped implant features a “natural sloped profile and inferior pole projection.” A variety of widths, heights and projections provide surgeons with more opportunities to create a customized result.

The Sientra shaped implant comes in 3 base shapes: round, oval and classic. The high-strength silicone gel is cohesive, so it cannot leak like saline implants or previous silicone implants. While they are designed to maintain their shape, they still offer the soft feel that women want from a silicone implant. Sizes range from 170 cc all the way up to 700 cc in volume.

The cosmetic surgery website RealSelf.com is one such place, where users support each other through their struggles. A recent post on the site highlighted 10 tips from various community members who have undergone gastric bypasses, a form of bariatric surgery that often precedes body contouring surgery.

1. Follow your doctor’s advice. Quit smoking, follow a set exercise routine, take your medication regularly, and whatever else your doctor recommends. Your body will thank you.

2. Do an honest self-evaluation. Sometimes it helps to take a close look at your body, habits, and routines. What do you want, what do you need, and what can you leave behind?

3. Have the right attitude. My best advice is to be practical but optimistic – “keep your head in the clouds and your feet on the ground.”

4. Drink a lot of water. H2O is beyond important. Put away those sugary drinks and pick up some cold, refreshing water, especially when exercising.

5. Don’t rush back into eating. After gastric bypass surgery, patients need to abide with liquid and puréed foods for some time. It’s important to ease the transition back into regular eating habits.

6. Change your eating habits for good. “Say to yourself, this food does not taste as good as skinny feels,” says RealSelf user Cindy.

7. Learn coping mechanisms. “You have to see someone and really spend serious time talking about what to expect when you have slimmed down,” says Cynthia J. “You will still have your issues with self worth but you will learn to have your coping mechanisms.”

8. Join a support group. Many doctors offer support groups where patients can talk about their experiences. This is especially important for bariatric patients, who have a long and rocky road to full recovery.

9. Realize that surgery is only a tool. Proper diet and exercise are still essential. You can still be unhealthy, even after a gastric bypass, if you don’t take the right steps.

10. Determine if it’s “worth it.” This is a question that every prospective patient has to ask. After massive weight loss, patients often find that they are left with excess skin, and require body contouring procedures such as the breast lift, body lift, and tummy tuck. FDA-Approved Silimed Breast Implants Are Safe According to Follow-up Study

A new follow-up study has reaffirmed the safety of Sientra’s Silimed brand silicone gel implants, which were approved in March by the FDA. Among the results of the study, which appears in the most recent issue of Plastic & Reconstructive Surgery, was a 99 percent rate of satisfaction among surgeons with the postoperative results.

The Sientra implants became the third FDA-approved silicone gel-filled breast implant in the United States when they were approved in March, joining implants manufactured by Allergan and Mentor. As one of the conditions of the approval, the FDA required Sientra to conduct post-approval studies. These would assess long-term safety and effectiveness outcomes, as well as the risks of rare complications.

The original approval for Sientra’s implants followed the largest silicone gel breast implant study to date. It made history for being the first approval for shaped silicone gel breast implants, which have been used worldwide, most notably in Brazil. This latest study presents the results of Sientra’s ongoing study through 5 years.

There are 1,788 women participating in the study, who were altogether implanted with 3,506 implants. These included:

  • 1116 primary augmentations
  • 363 revision-augmentations
  • 225 primary reconstructions
  • 84 revision-reconstruction subjects

During their follow-up appointments, the participants have physical evaluations, and complications are recorded at each visit. The effectiveness of the implants was measured by bra cup size, as well as ongoing assessments of subject satisfaction and quality of life.

What are the results after five years?

Overall, the risks involved with the Sientra implants are comparable to the implants already manufactured by Allergan and Mentor:

  • rupture: 1.8%
  • contracture: 9%
  • reoperation: 23.8%

The authors show that over 99 percent of surgeons reported satisfaction with the postoperative results. As well, subject satisfaction remained high, even five years after their surgeries. As a result, the authors argue, the five-year results “continue to provide a comprehensive safety and effectiveness profile.”

Is Breastfeeding the Only Cause Of Droopy Breasts?

As we all know, pregnancy and breastfeeding can have a big effect on women’s breasts – leaving them feeling saggy and deflated. But are pregnancy and breastfeeding the only causes of drooping breasts? A new study in the Aesthetic Surgery Journal says no. In fact, there are a number of different factors that can lead to droopy breasts over time.

Like many studies, this one began by comparing identical twins. In 2009 and 2010, 161 pairs of identical female twins attending the Twins Days Festivals in Ohio had their breasts photographed. Researchers compared the photographs, while accounting for the twins’ different life experiences. They found several factors that can result in drooping breasts.

Your breast skin and tissue can become stretched due to hormonal and weight changes associated with pregnancy, breastfeeding, or simply weight gain. Once the skin becomes stretched, the breasts sagging droop. The researchers also found that smoking and alcohol use can contribute to saggy breasts.

Fortunately, these lifestyle factors can usually be mitigated. By quitting smoking and cutting down on alcoholic drinks, you can reduce your chances of getting saggy breasts in the future – as well as improve your health more generally! Maintaining a stable body weight is also a great way to keep a sleek figure.

But what about breastfeeding and pregnancy? While most women choose the joy of a new baby, regardless of bodily changes, there are techniques that you can use to mitigate stretching during this time. An article on the LiveStrong website highlights just a few handy tricks:

  • Massaging your breasts while bathing can increase circulation to your skin.
  • Daily multivitamin supplements can help your skin’s elasticity.
  • Moisturizers containing vitamin E and collagen can promote your skin’s resilience.
  • A well-balanced diet and regular exercise can prevent excess weight gain during your pregnancy.
  • By drinking lots of water, you will help keep your skin moist and elastic.

Of course, once your breasts have been stretched, that doesn’t have to be the end of the road. You still have a few options to restore the curves of your youth. With a breast lift, sagging breasts can be lift and reshaped, so that they appear youthful and perky. Many women combine a breast lift with tummy tuck surgery for overall enhancement after pregnancy.

What is Revision Breast Surgery?

Breast augmentation or a Breast Lift can be an incredible surgery that transforms your physical appearance and improves your confidence and self-esteem.  The surgeons of Beautologie are extremely successful in providing excellent results with breast enhancement surgery.

But what happens if you inadvertently went to a non-qualified doctor and things went “wrong”?  Recent news stories in the Bakersfield Californian point out why it is so important to choose a real plastic surgeon, not one that has only weekend courses in these technically challenging procedures.

At our Beautologie offices in Bakersfield, Fresno and Malibu we are frequently consulted to help repair a breast surgery “disaster”, often one that was done under local anesthesia, or by a non fellowship trained plastic surgeon that does not have appropriate training.  In these cases, a revision may be required to address issues such as loss of volume, sagging skin, the positioning of the implant, or an infection. In many cases, the patient has lost the implant completely leaving loose scarred tissue, or has had multiple attempts at repair which have caused more damage.

In any type of revision surgery, identifying the cause of the problem and performing the right type of surgery to correct it is extremely important. Only a knowledgeable and highly experienced plastic surgeon should be consulted regarding a breast augmentation revision to ensure the best possible outcome to the procedure. These surgeries should only be performed by someone that has extensive revision surgery experience.

We have the experience and the skill to help you repair a bad experience.  Sometimes, the revision may require multiple procedures and may never turn out “perfect”. This is why it is important to choose a qualified surgeon at first.   Get it done right from the beginning, and you can avoid a costly and long procedure to fix a deformity.

Rarely a breast revision surgery will be needed following an augmentation due to the contraction of the capsule. A capsule is the normal scar tissue that will cover the implant after it has been placed in your body.  With some patients, the capsule will tighten and squeeze the implant.  One way to correct the shrinking is to open the capsule to create more space for the implant.

Another way is to remove the capsule completely.  Often, if the capsule is mild, we can prescribe medication that may help eliminate the capsule without surgery.  There are many theories as to why a capsule shrinks, including infections, trauma or even the flu.  Capsular contractions are more common when the implant has been inserted above the muscle (under the skin. The best location for an implant is behind, or partially behind, the muscle, and is the kind of procedure that we typically perform to avoid capsular contracture.  Also, choosing the under the breast incision helps to prevent complications.

Breast revision surgery may also be necessary if the implant that was used during your augmentation surgery was not round.  A natural or tear-drop shaped implant can rotate and can cause deformities.  Our office typically recommends round silicone and saline implants to prevent this from occurring.  We also know how to correct problems and deformities that have resulted from the use of implants that have rotated because of their shape.  The best way is to replace the implant to a round implant.

A breast implant will need to be removed or replaced if it has ruptured or starts leaking.  A ruptured implant is extremely rare, but does occasionally happen.  When a saline implant ruptures, the saline will leak rapidly and cause a deflated looking appearance.  The rupture of a silicone implant may not be noticeable for a long period of time.

If your implant ruptures and you chose to have it permanently removed, a breast lift may an appropriate solution to tighten the area.  Implant exchange is typically an “easier” procedure than the original surgery to place the implant, and the recovery is quicker.
