Refund and Financial Policies

SURGERY

This quote is valid for ten (10) calendar days. All surgical procedures require a $1,000 non-refundable reservation fee which allows you to secure this treatment plan for up to (1) year and covers the cost of your preoperative expenses (including scheduling, preoperative appointments, teaching, standard blood work, internal medical clearance, etc.). Your deposit is always NON-REFUNDABLE and NON-TRANSFERABLE, whether a date for surgery is booked or not.  Medications, additional blood work, garments, and any outside doctor visits or facilities (ie. a hospital) are NOT included in this treatment plan and are your separate financial responsibility to pay and obtain these prior to your procedure if requested.

CANCELLATION/POSTPONEMENT OF SURGERY POLICY: You must have surgery within (1) year of paying your deposit, or you will forfeit all paid amounts. Any unpaid surgical balances are due at your pre-operative visit, which is usually 30-60 days before surgery. Please note we do not accept personal checks or cash for this payment. We understand that plans can change, and personal circumstances may impact your ability to proceed with the scheduled procedure. Our providers have committed time and resources in securing this time for you. The following policy outlines the fees associated with cancelling or postponing your procedure.

1. Cancellation notification to our office 30 days or more before your scheduled procedure will allow a refund of all payments except the deposit ($1000 per procedure scheduled). If rescheduling to another date, this deposit can be held on your account and applied to rescheduling a new surgery date that is within 12 months of your cancelled date.  Refunds to a form of payment that is different than the original form of payment will result in an additional 5% (or higher if using financing) fee.
2. Cancellation notification to our office within 30 days of scheduled surgery will result in no refund. To be added back to the surgery schedule a $1000 rescheduling fee will be required unless our medical clearance provider has determined it to be a valid medical reason for rescheduling.
3. Cancellation on day of surgery: If you cancel on day of surgery, the entire amount is non-refundable. If our anesthesia provider and medical team cancels your surgery for any reason and it is determined you cannot reschedule permanently by our medical director, then you will be refunded your paid amount minus $1000 to partially cover the preoperative and operative expenses of that day.

PRE-OPERATIVE CLEARANCE: If you are over 50 years of age, or if your surgeon recommends, you are responsible to obtain a pre-operative clearance from our office for your own safety.  If you are determined to not be a candidate for surgery by our doctor, we will refund your deposit minus $250 to cover our preoperative, lab work and physician expenses.

SMOKING (TOBACCO, MARIJUANA, VAPE, ETC)/ ILLICIT DRUG USE POLICY: You must stop smoking and not use any drugs a minimum of ONE MONTH BEFORE surgery.  For your own safety, we perform a urine test at your pre op and surgery visit to confirm. If it is positive, we must cancel your surgery because you cannot undergo anesthesia, and the above cancellation rules apply.

By signing this treatment plan you agree that you understand all information above. We reserve the right to not perform these procedures and cancel this quote at any time.

All treatment plans are valid for 90 days or 3 months. Any treatment done after the initial 90 days of this treatment plan will be subject to current/updated pricing (Excluding package treatments).
All sales are final.

All payments made are for one treatment only - no-repeat treatments are included in the above payment unless a package is specifically stated.

Any No Show or Late Cancellation (less than 48-hour notice) will be subject to a $25.00 Fee.
